Self-Adovacy, Person-Directed Planning, and Self Determination

>>Transition refers to the time in which a
student becomes an adult and the discussion about what you will be doing post-graduation. Transition begins at school. It involves you, your parents, your teachers, and the team to discuss what
you’d like to do after graduation. It’s important to understand
both your needs and wants. Keep in mind that at age 18, the law
considers you an adult with all the rights and responsibilities of every other adult. If you have any questions, now’s a good
time to discuss what you’d like to be doing after graduation, whether it be
going to college, going to work, where you might be living and whatnot. There are three ways to plan your future. They include self advocacy, self-determination,
and person directed planning. Self advocacy is knowing what you want,
what you don’t want, what you need, your areas of skill, what your interests are. It’s important to ask questions, to be
assertive in your [inaudible] meetings. Self-determination refers to knowing what you
already want and applying those to a plan. You may be discussing goals, making
decisions, and solving problems as they arise. It means knowing your abilities
and being able to follow your plan. Person directed planning refers actually to
taking action based on the choices you’ve made. It is empowering you to reach out to
agencies in order to meet your needs. Both you and your LAR or you’re legally
authorized representative can work together in order to support your
goals as well as decisions. It’s important that you know
your support services by now. Everything you’ve been using
up until this point, what type of communication needs
are important for you in meetings? Also being able to identify those who’ve
supported you up until this point. That’s all part of your LAR. The timelines of when you turn
14 or before are as follows. Between ages 14 to 16 are as follows. Between ages 16 up to 18 or
when you turn 18 are as follows. Once you’ve completed school,
the following applies.
